 The National Youth Adocacy Service has published a report called Making Choices - A study into young disabled people's experiences of transition in Torfaen.
The report looks at children and young people's experiences of transition into adulthood and provides a snap-shot of their views on the support they received at different stages of the transition process.
The report is available to download from the National Youth Adocacy Service website.
